How to Register for a Course

From this page you can learn how to register for either a program course or a continuing dental education (CDE) course. Program courses must be registered for either by mail or in person, while CDE courses can be registered for by mail, in person, or online. Click on the links or scroll down for further details.

If this is your first time registering or making a purchase with us, please carefully read the information below. Register for a Program Course

In order to register for a program course (those that help you obtain your dental x-ray license, become a CDA

certificant, or become an RDA) you must complete a registration form. Links to these forms are below. Simply click on the appropriate link which will open a new window or tab in your browser and fill in the form. When you are finished, please print out the form and mail it in along with the appropriate payment or deposit.

More specific instructions are provided on the actual registration forms, including whether a deposit is required or if payment in full is required up front.

If you would like to register by mail and are paying by check or money order please enclose your payment made out to Dental Assistant Services along with the completed registration form and mail it to the address at the top of the form. If you would like to pay by cash or credit card, these transactions must be made in the office. Please bring your completed registration form with you. We accept Visa, MasterCard, and Discover. Of course if you prefer you can also drop off your registration form in person and pay by check or money order.

If you decide to register in person, you may want to call ahead to make sure that someone will be available before stopping in. Unfortunately at this time, we cannot accept credit cards over the phone and do not offer online registration for program courses.

Register for a Continuing Dental Education (CDE) Course

CDE courses can be registered for either by mail, in person, or online. See below for details.

Registering by mail or in person

In order to register by mail or in person you must complete a CDE registration form. Simply click on the link which will open a new window or tab in your browser and fill in the form. When you are finished, please print out the form and mail it in along with the appropriate payment. The address is at the top of the registration form.

Registering online

If you register online you do not need to complete and mail in a registration form as described above. Registration is completed through PayPal, a secure third-party website. You do not need to have a PayPal account in order to register for or purchase a course. Click here to learn more about PayPal.

All major credit cards are accepted including Visa, MasterCard, American Express, and Discover. If you have a PayPal account you can also use your PayPal account balance.
